Job Opportunity

BayCare HomeCare is seeking a dedicated and compassionate Home Health RN to join our team. As a key member of our multidisciplinary team, you will provide high-quality patient care in the comfort of their own homes.

Responsibilities:

  • Oversee the activities of the multi-disciplinary team that provides care to patients
  • Monitor the provision of therapeutic services through written and verbal communication with all disciplines
  • Ensure that all visits scheduled and provided are authorized and covered by physician orders
  • Review patient visits weekly and provide a plan to the Patient Care Supervisor and team assistant
  • Inform the scheduler of any schedule changes and communicate caseload and patient care issues to the Home Care Supervisor

Requirements:

  • BLS (Basic Life Support)
  • RN (Registered Nurse) License
  • Driver's License State of Florida (Florida driver's license is required within 30 days of hire)
