Key Responsibilities

The Hospice Nurse Practitioner plays a crucial role in providing expert consultation in palliative care, symptom relief, and supportive services tailored to the needs of hospice patients as directed by their referring physician.
  • Conducts in-person visits to eligible hospice patients within designated timeframes and communicates clinical observations to the Hospice Medical Director.
  • Collaborates with the Hospice Medical Director as necessary, updates the primary physician on recommended services, and works alongside other medical professionals as required.
  • Develops and coordinates individualized care plans. Accurately documents patient issues, goals, interventions, and responses to hospice services. Engages with patients, families, attending physicians, and interdisciplinary team members to ensure comprehensive care.
  • Educates and guides patients and families in self-care practices when suitable.
  • Keeps precise and relevant clinical records regarding patient conditions. Reports any changes in patient status and identified needs to the Administrator and Hospice Medical Director.

Qualifications

License Requirements
  • Current CPR certification is mandatory.
  • Active certification as a Nurse Practitioner or Clinical Nurse Specialist in the relevant state.
  • Certification from ANCC or AANP is required.
  • Possession of a valid driver's license, vehicle insurance, and access to reliable transportation.
  • NPI Number is necessary.
  • DEA registration with prescribing authority.
  • PECOS enrollment certification is required.
  • CAQH enrollment is necessary.
  • Medicare and Medicaid numbers (the latter is preferred but not mandatory).

Experience Requirements

  • Minimum of one year of experience as a Nurse Practitioner in chronic disease management, family practice, or primary care.
