Key Responsibilities

The Hospice Nurse Practitioner offers expertise in palliative care, symptom alleviation, and supportive services tailored to the needs of hospice patients as directed by the referring physician.
  • Conducts in-person visits within designated timeframes to eligible hospice patients and communicates clinical observations to the Hospice Medical Director.
  • Collaborates with the Hospice Medical Director as necessary, updates the primary physician on recommended services, and engages with other healthcare providers as needed.
  • Develops and coordinates the care plan. Records issues, relevant goals, interventions, and patient/family responses to hospice services. Works in partnership with the patient/family, attending physician, and other members of the interdisciplinary team to deliver comprehensive care.
  • Educates and guides the patient/family in self-care practices when appropriate.
  • Keeps precise and pertinent clinical documentation regarding the patient's status. Reports on patient conditions and any identified needs or concerns to the Administrator and Hospice Medical Director.

Qualifications and Experience

Licensure Requirements
  • Valid CPR certification is mandatory.
  • Current certification as a Nurse Practitioner or Clinical Nurse Specialist in the relevant state.
  • Certification from ANCC or AANP is required.
  • Possession of a valid driver's license, vehicle insurance, and access to reliable transportation or public transit.
  • NPI Number is necessary.
  • DEA registration with prescribing authority.
  • PECOS enrollment certification is required.
  • CAQH enrollment is necessary.
  • Medicare number is required.
  • Medicaid number (preferred, not mandatory)

Experience

  • More than one year of Nurse Practitioner experience in chronic disease management, family care, or primary care settings.
