Hospice Care Specialist

At HealthONE Hospice & Family Care, we are committed to providing compassionate and high-quality care to our patients and their families. As a Practical Nurse - Hospice Care, you will play a vital role in delivering skilled nursing care to patients in their homes.

Key Responsibilities:

Administer quality, skilled nursing care to patients in the home
Work in accordance with physician orders and the Plan of Care under the direction and supervision of a Registered Nurse and/or Director, Professional Services
Perform an ongoing assessment during each visit and document data in patient medical records

Requirements:

Basic Cardiac Life Support must be obtained within 30 days of employment start date
Licensed Practical / Vocational Nurse (LPN / LVN)
Drivers License
Vocational School Graduate
Occasional/ Intermittent Required
1 years experience Required Years of Experience
