Job Summary:
The HIV Prevention Coordinator role at the Joseph P. Addabbo Family Health Center, Inc. involves coordinating care for patients, arranging linkages to care, and establishing professional connections with HIV/AIDS and health services providers.

About the Job:
This position requires a strong understanding of HIV prevention, PrEP, and available social services. The ideal candidate will have experience working with PLWHA, LGBTQIA+, and communities of color. They must be comfortable working independently and collaboratively in a remote environment, possess excellent organizational and interpersonal skills, and be able to handle multiple assignments at any time. A bachelor's degree in Health Education, Public Health, or a related field is preferred, but relevant work or volunteer experience may be considered in lieu of a degree.

Responsibilities:
* Provide culturally competent education on HIV prevention, PrEP, and available social services
* Perform light case management for patients, including appointment reminders and assisting those prescribed PrEP and PEP in developing and adhering to a medication management schedule
* Coordinate linkages to care for clients seeking treatment adherence support, housing, entitlements, harm reduction education, and health insurance
* Ensure appropriate follow-up for client referrals to external resources such as substance use treatment, mental health services, restorative justice, legal services, and nutrition support
* Conduct sexual and social history screenings for linkages to appropriate care/services
* Reduce barriers to obtaining clinical/care services by working with providers and other clinic staff to ensure that HIV care is treated as primary care

Requirements:
* 2 years of case management experience preferred
* Experience working with PLWHA, LGBTQIA+, and communities of color preferred
* Computer literacy with proficiency in MS Word, Excel, and PowerPoint, as well as electronic medical records preferred
* Demonstrated organizational, interpersonal, oral, and written communication skills and the ability to handle multiple assignments at any time

Salary: $60,000 - $80,000 per year
