Job Summary

We are seeking a Spiritual and Grief Counselor to join our interdisciplinary team. The successful candidate will provide spiritual and grief support to patients and families, develop and implement individualized care plans, and work collaboratively with the patient's designated faith and spiritual community.

Key Responsibilities
  • Respond to referrals from clinical teams by contacting patients and/or families to establish a counseling relationship and expectations.
  • Perform spiritual and grief assessments, interventions, and care planning to meet the individual needs of patients and/or families.
  • Engage in ongoing assessment of care plans through interdisciplinary review in accordance with Federal and State Guidelines.
  • Provide bereavement support, including developing individualized bereavement plans of care and connecting bereaved individuals with community resources.
  • Lead community grief groups and memorials, and participate in the planning and coordination of Agrace community bereavement events.
Requirements
  • Master's Degree in Social Work, Counseling, or other related counseling degree, or equivalent combination of education and work experience.
  • At least one unit of Clinical Pastoral Education (CPE) strongly preferred.
  • Two or more years' counseling experience preferred.
  • Valid driver's license, auto insurance, and access to a reliable vehicle.
  • Experience providing counseling and/or spiritual care in both group and individual settings.
  • Experience working with grief as related to death.
  • Intermediate Microsoft Office Suite skills.
  • Ability to work effectively and cooperatively with others.
  • Ability to communicate effectively verbally and in writing.
  • Ability to be flexible in time and task to meet organizational needs.
  • Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property.
