Job SummaryWe are seeking a Spiritual and Grief Counselor to join our interdisciplinary team in Madison/Middleton. The successful candidate will provide spiritual and grief support to patients and families, working collaboratively with the clinical team to develop and implement individualized care plans.
Key Responsibilities- Respond to referrals from the clinical team, establishing counseling relationships and expectations with patients and families.
- Perform spiritual and grief assessments, interventions, and care planning to meet the unique needs of patients and families.
- Engage in ongoing assessment of care plans through interdisciplinary review, in accordance with Federal and State Guidelines.
- Work collaboratively with patients' designated faith and spiritual communities, and refer to other community resources as needed.
- Develop and implement individualized bereavement plans of care to address the full scope of survivor needs.
- Provide updates to the interdisciplinary team regarding bereaved individuals and connect them with community resources to assist in the grieving process.
- Lead community grief groups and memorials, developing and implementing support groups for Agrace families and community members.
- Participate in internal work groups, meetings, and committees relative to position responsibilities.
- Master's Degree in Social Work, Counseling, or a related field, or an equivalent combination of education and work experience.
- At least one unit of Clinical Pastoral Education (CPE) strongly preferred.
- Two or more years' counseling experience preferred.
- Valid driver's license, auto insurance, and access to a reliable vehicle.
- Experience providing counseling and/or spiritual care in both group and individual settings.
- Experience working with grief related to death.
- Intermediate Microsoft Office Suite skills.
- Ability to work effectively and cooperatively with others.
- Experience using industry-specific software.
- Ability to communicate effectively verbally and in writing.
- Ability to be flexible in time and task to meet organizational needs.
- Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property.
- Ability to work at various Agrace locations.
- Ability to use proper body mechanics and standard precautions.
