Position Summary
The Behavioral Health Care Coordinator plays a crucial role in our Care Management program, focusing on individuals who frequently utilize services but lack effective connections to necessary support systems. This position primarily addresses the needs of uninsured or underinsured clients who exhibit high usage of acute care services, including crisis stabilization and inpatient treatment. The Care Coordinator will evaluate individual needs, develop a tailored care or treatment plan, and conduct outreach to engage clients referred from various facilities.


Key Responsibilities

  • Accountability: Act as the primary contact responsible for coordinating services and ensuring a holistic approach to meet individual needs.
  • Engagement: Establish trust and rapport with clients by providing outreach and encouraging participation from their support networks.
  • Assessment: Utilize standardized tools to determine the appropriate level of care required.
  • Collaborative Planning: Develop individualized, strength-based care plans that prioritize the values and preferences of the client.
  • Community Integration: Ensure services are delivered in inclusive and accessible environments that promote community involvement.
  • Healthcare Coordination: Integrate services across various sectors, including physical health, behavioral health, and social services.
  • Information Sharing: Comply with legal standards to share information among relevant service providers and support systems.
  • Transition Support: Facilitate introductions between clients and their care coordinators to ensure a smooth transition of care.
  • Cultural Competence: Respect and incorporate the cultural values and beliefs of clients into the care process.
  • Outcome Monitoring: Align care plan goals with measurable success indicators and adjust plans as necessary.
  • Symptom Stabilization: Support clients in stabilizing their mental health through effective care coordination.
  • Advocacy: Champion the needs of clients by coordinating necessary services and monitoring satisfaction.
  • Outreach: Engage with individuals referred from various facilities to inform them about available services.
  • Ongoing Communication: Maintain regular contact with clients to ensure continuity of care.
  • Provider Outreach: Inform service providers about available resources at critical points in the care system.

Qualifications

  • A Bachelor's Degree in Social Work or a related Human Services field from an accredited institution is required.
  • At least one year of experience in human services or a mental health-related role is necessary, with a preference for experience with adults facing mental health challenges.
  • Proficiency in electronic health records and patient information systems is expected within three months of employment.
  • Strong skills in Microsoft Office and internet navigation are required.
  • A valid driver's license and appropriate insurance coverage are necessary for travel to various locations.
  • Excellent communication skills are essential for effective interaction with clients and other stakeholders.
