The RoleWe're seeking an experienced RN Care Manager to join our team. As a critical part of our care delivery model, you'll work closely with patients, families, and local care teams to coordinate services and optimize care outcomes.
Key Responsibilities:
- Develop personalized care plans addressing clinical and non-clinical needs
- Conduct regular assessments to detect potential risks and refer matters to appropriate channels
- Initiate proactive outreach to patients and clinics as per program guidelines
- Manage a diverse patient panel while adhering to NCQA standards
- Communicate updates to the care team and family members
- Liaise with dialysis facilities and maintain accurate program software records
- Generate and evaluate reports using Strive data platforms
- Provide culturally sensitive care to meet the diverse needs of Strive patients
To be successful in this role, you'll need:
- An Associate's degree from an accredited school of nursing
- A current Registered Nurse (RN) license in the state of practice
- 3+ years of combined related education, experience, or certification
- Current BLS or CPR Certification
- Efficient and reliable transportation
We offer a competitive salary range of $74,000 - $90,000 annually, along with a comprehensive benefits package, including:
