About the Role

We are seeking a highly skilled and compassionate Physician to join our team at JM Chandler Dream Builders. As a Geriatric and Post-Acute Care Specialist, you will be responsible for providing high-quality medical care to seniors in assisted living, life plan communities, independent living, skilled nursing, and long-term care facilities.

Key Responsibilities
  • Diagnose and treat a variety of diseases and injuries in geriatric and post-acute care settings
  • Develop and implement treatment plans for patients with physical and/or cognitive impairments and disabilities
  • Monitor ongoing progress and health of patients, and make necessary changes to treatment plans
  • Provide adaptive tools to improve the overall health and physical functioning of patients
  • Counsel patients and their families to help them adapt to lifestyle changes
Requirements
  • Doctor of Medicine (M.D) or Doctor of Osteopathy (D.O.) degree
  • Previous experience in geriatrics and post-acute care settings, including SNF, IPR, and LTACH
  • Proficiency in documenting and billing in any setting above, or in the hospitalist setting
  • Current unrestricted license to practice medicine in the State of employment
  • Certification by The American Board of Physical Medicine and Rehabilitation (ABPMR)
  • Current DEA certificate (or in progress)
