Job Title: Registered Nurse

Element Care is seeking a skilled Registered Nurse to join our team. As a member of our interdisciplinary team, you will play a vital role in providing high-quality care to our participants.

Key Responsibilities:
  • Provide direct care to participants, including assessments, care planning, and implementation.
  • Collect and analyze medical and social information to ensure comprehensive decisions are made.
  • Monitor clinic care and service delivery to ensure conformance with quality assurance and operational policies.
  • Coordinate with the interdisciplinary team to develop comprehensive care plans for each participant.
  • Actively participate in implementing a summary on each client to help determine Nursing Home Level of Care.
  • Ensure all tests, lab work, and diagnostic studies are carried out as ordered by the Physician.
  • Coordinate medication, including stocking, ordering, and administering medication in the PACE Center.
  • Provide education and consultation to participants and caregivers on care plans, nursing treatments, and medications.
  • Communicate participant's condition to primary caregivers, physician, and multidisciplinary team.
  • Build strong relationships with participants, caregivers, families, and community providers to facilitate optimal care and financial oversight.
  • Participate in after-hour visits.
Requirements:
  • Registered Nurse currently licensed in the Commonwealth of MA.
  • Minimum of two (2) years experience with the geriatric population.
  • Proficient in the use of Microsoft Office tools.
  • Ability to travel locally on a frequent basis in a personal vehicle.
  • Ability to quickly recognize situations/problems before they become acute, and to implement effective solutions and understand consequences.
