About the Role
- Deliver individualized, goal-directed patient care that prioritizes patient-centeredness and family involvement.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and optimal patient outcomes.
Key Responsibilities
- Patient Assessment and Care Planning
- Conduct comprehensive patient assessments to identify needs and develop personalized care plans.
- Collaborate with registered nurses to prioritize patient care and ensure timely interventions.
- Provide direct patient care services, including medication administration and wound management.
- Facilitate communication between patients, families, and healthcare teams to promote informed decision-making.
- Maintain accurate and detailed records of patient interactions, including progress notes and treatment plans.
- Submit timely reports on patient status and care outcomes to facilitate data-driven decision-making.
Requirements and Qualifications
- Education and Licensure
- Graduate of an accredited licensed practical nursing program.
- Holding a current LPN license in the state of South Carolina or a compact state is mandatory.
- A minimum of six months of experience as a licensed practical nurse is required.
- Demonstrated expertise in ambulatory care settings, preferably in an outpatient setting.
- Proficient in using electronic health records and other medical software applications.
- Excellent communication, teamwork, and time management skills.
- Current certification in Basic Life Support (BLS) for Healthcare Providers is a plus.
