Nurse Care Manager

As a skilled and compassionate Registered Nurse, you will be responsible for providing high-quality patient care and management to an assigned group of patients. You will collaborate with the Nurse Manager and other healthcare team members to achieve optimal patient outcomes.
Key Responsibilities- Assess and monitor the overall needs of patients and families, documenting nursing care and teaching consistent with documentation guidelines.
- Develop and maintain individualized patient plans of care, reflecting the full scope of practice and addressing patient and family changing needs and goals.
- Deliver care in a culturally sensitive manner, preserving patient autonomy, dignity, and rights.
- Communicate effectively with patients, families, and healthcare team members, including triage of incoming calls, to ensure continuity and coordination of care.
- Act as a patient and family advocate, identifying and addressing their needs and concerns.
- Initiate patient discharge planning and facilitate a smooth transition to the next point of service or home.
- Evaluate patient outcomes and educational needs, developing and implementing appropriate teaching strategies or making referrals as needed.
- Associate's Degree in Nursing (required) or Bachelor's Degree in Nursing (preferred).
- Current licensure as a Registered Nurse in the State of Michigan.
- Basic Life Support (BLS) certification within 90 days of hire.
- Advanced Cardiovascular Life Support (ACLS) certification within 90 days of hire (for specific roles).
- Pediatric Advanced Life Support (PALS) certification within 180 days of hire (for specific roles).
