The incumbent is responsible for facilitating communication, coordinating services, addressing obstacles, and optimizing resource allocation while ensuring high clinical quality and managing costs.

Key Responsibilities:
As a Registered Nurse Care Coordinator, your expertise will encompass the following:

Conduct thorough assessments of patients' health requirements, including health status, behaviors, functional levels, psychosocial factors, and available support systems to identify potential needs.

Collaborate with primary care physicians and the patient care team to establish comprehensive care plans.
Provide essential health education to patients.
Identify at-risk patients for proactive intervention.
Analyze data to identify high-risk patients and present findings to the care team.

Refer patients to various resources, including nutrition, social work, rehabilitation, behavioral specialists, diabetes education, and wellness workshops.

Engage with patients who have utilized emergency services or been hospitalized post-discharge to ascertain the reasons for their visits and collaborate with them to create plans to avoid future occurrences.

Coordinate care for complex cases or patients consulting multiple specialists.

Maintain accurate and up-to-date documentation of patient assessments and care plans to ensure seamless integration of information for the healthcare team, thereby supporting ongoing quality care in line with evidence-based practices.

Analyze patient care trends and actively collaborate with the care team to enhance overall care quality and efficiency.

Utilize registry data to identify service gaps and initiate interventions.
Demonstrate critical thinking for effective problem-solving and prioritization.
Lead and participate in patient care conferences.
Manage patient panels alongside physician office staff to identify suitable patients and measure outcomes.
Collaborate closely with physicians and office personnel.

Advocate proactively for patient care issues to ensure that the quality and type of care are tailored to meet the unique needs of each patient and family.

Work in partnership with payers and external agencies to promote a patient-centered care model.

Engage in committees and initiatives related to the development of Medical Homes, Accountable Care Organizations (ACO), and other insurance entities.


Minimum Qualifications:
Bachelor’s Degree (BS/BA/BSN)
Current RN license in the state of practice
A minimum of two (2) years of clinical experience in an outpatient environment or two (2) years of experience coaching individuals with complex and/or chronic conditions.

Preferred Qualifications:
Previous experience in care coordination within a community setting or primary care practice
Experience in program development, operations management, and/or supervisory roles.

Physical Requirements:

Continuous need for employees to see and read information, labels, assess patient needs, operate monitors, and identify equipment and supplies.

Frequent interactions with patients, care providers, and visitors necessitate effective verbal communication and the ability to quickly comprehend spoken information, alarms, needs, and issues, especially in emergency situations.

Manual dexterity is required to manipulate complex and delicate equipment with precision. This includes regular computer use and typing for documenting patient care, accessing necessary information, and preparing medications.
