Baylor Scott & White Health is seeking a skilled Care Manager RN to oversee patient care and ensure optimal outcomes. As a key member of our healthcare team, you will be responsible for developing and implementing patient care plans, coordinating services, and promoting healthy behaviors.
Key Responsibilities- Develop and implement patient care plans in collaboration with the healthcare team
- Coordinate services to ensure seamless care transitions
- Provide education and support to patients and families to promote healthy behaviors
- Monitor and evaluate patient progress, making adjustments to care plans as needed
- Collaborate with hospital, long-term care, and home health representatives to ensure comprehensive care
- Associate's degree in nursing
- 2 years of experience in care coordination or a related field
- RN license required
