Key Responsibilities- Provide comprehensive primary care services to patients residing in skilled nursing facilities
- Rounding and managing clinical care of the population at the assigned SNF
- Prescribing medications and treatments for residents residing at SNFs
- Timely documentation of all patient care/encounters
- Available for phone/secure messaging consultations with facility nursing staff
- Reliable transportation required
- Timely attendance is imperative
- Ability to work in a fast-paced environment
- Strong communication and interpersonal skills
