Theoria Medical is seeking a skilled Nurse Practitioner to join our team in providing high-quality, patient-centered care to residents in skilled nursing facilities. As a key member of our team, you will be responsible for rounding and managing the clinical care of our patients, prescribing medications and treatments, and providing comprehensive primary care services.
Responsibilities- Rounding and managing the clinical care of patients in skilled nursing facilities
- Prescribing medications and treatments for residents
- Providing high-quality, patient-centered, comprehensive primary care services
- Timely documentation of all patient care/encounters
- Availability for phone/secure messaging consultations with facility nursing staff
- Reliable transportation required
- Timely attendance is imperative
- Ability to work in a fast-paced environment
- Strong communication and interpersonal skills
