Job Summary:

We are seeking a skilled Healthcare Navigator to join our team at Ascension. As a key member of our care team, you will play a vital role in ensuring the highest level of patient care and satisfaction.

Key Responsibilities:

  • Coordinate patient care throughout the continuum of care, ensuring seamless transitions and optimal outcomes.
  • Act as a liaison between management staff, administration, physicians, managed care companies, community organizations, and other customers, fostering strong relationships and effective communication.
  • Develop and implement strategies to promote quality improvement and educational efforts, driving excellence in patient care.
  • Compile and analyze data, track outcomes, and make recommendations for process improvement, ensuring data-driven decision making.
  • Provide referrals and support for follow-up care for patients who are uninsured or underinsured, ensuring access to necessary resources.

Requirements:

  • Registered Nurse credentialed from the Oklahoma Board of Nursing required.
  • Diploma from an accredited school/college of nursing or required professional licensure at time of hire.
