About the Role

We are seeking a compassionate and skilled Home Health Aide to join our team at Hackensack Meridian Health. As a Home Health Aide, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Assist patients with personal care activities, such as bathing, dressing, and grooming, while ensuring their safety and dignity.
  • Monitor and report any changes in patient condition to the registered nurse, and document all interactions and care provided.
  • Assist patients with transfers, ambulation, and range of motion exercises, while maintaining a safe and supportive environment.
  • Perform general housekeeping duties, such as changing bed linens and assisting with light housekeeping tasks.
  • Assist patients with self-administration of oral medications and remind them to take their medications as prescribed.
  • Prepare simple meals and assist patients with eating, as needed.
  • Assist with ostomy care, including changing colostomy bags and performing peri-care.
Requirements
  • N.J. Board of Nursing CHHA Certification
  • Valid driver's license
  • Good customer service skills
  • Home care experience preferred
