Job Summary

At Hackensack Meridian Health, we are seeking a skilled Registered Nurse Home Care Specialist to join our team. As a key member of our Home Care team, you will provide professional nursing care to patients in their homes, ensuring their comfort, safety, and well-being.

Responsibilities:

  • Assess patient needs and develop individualized care plans to meet their unique requirements.
  • Provide high-quality nursing care, including medication administration, wound care, and education on disease management.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and effective communication.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in ongoing education and training to stay current with best practices and industry developments.

Requirements:

  • Associate's Degree in Nursing or Diploma Certificate in Nursing.
  • At least six (6) months RN experience or at least 1 year of LPN experience, preferably in home care or hospice.
  • Current NJ State Professional Registered Nurse License and AHA Basic Health Care Life Support HCP Certification.
