About the Role:

CenterWell Primary Care is seeking a skilled Medical Specialist to join our team. As a Medical Specialist, you will play a vital role in providing proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.

Key Responsibilities:

  • Provide high-quality, patient-centered care to seniors, focusing on whole-person health and addressing physical, emotional, and social wellness.
  • Work collaboratively with integrated care teams, including physicians, nurses, and behavioral health specialists, to deliver compassionate, personalized care.
  • Develop and implement effective treatment plans, taking into account patient preferences and values.
  • Stay up-to-date with the latest medical research, guidelines, and best practices to ensure evidence-based care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.

Requirements:

  • Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
  • Graduate of an accredited MD or DO program from an accredited university.
  • Strong clinical skills and experience in outpatient medicine, continuity of care, health maintenance, and disease prevention.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
