As a Hospice RN/ RN Case Manager, you will report directly to the Administrator, Executive Director or Patient Care Manager Senior and maintain primary control and professional management of each patient. You will act as the primary liaison between physician, patient/family and hospice team, providing direct patient care services and supportive care to the patients family.

Key responsibilities include empowering patients caregivers by educating them on care for their loved one, managing and collaborating with an interdisciplinary group, evaluating patient and family needs, and assisting in the preparation and execution of the plan of care for each patient.

To be successful in this role, you will need to be compassionate, kind, and empathetic to your patients and their families. You will also need to be comfortable providing care wherever the patient calls home, demonstrate flexibility and composure while supporting others in a time of crisis, and have a RN license in the state of requested employment (or eligibility to obtain).
