Hospice Care Registered Nurse Opportunity

BAYADA Home Health Care is seeking a compassionate and skilled Registered Nurse (RN) to join our hospice care team in South Burlington, Vermont.

Job Summary

We are looking for a dedicated and experienced RN to provide high-quality care to our clients in their homes. As a hospice nurse, you will work closely with our interdisciplinary team to deliver palliative care and support to individuals and their families.

Responsibilities
  • Provide skilled nursing care to clients in their homes, including administration of medication, wound care, and treatments
  • Develop and implement individualized care plans in collaboration with the client's healthcare team
  • Conduct thorough assessments and evaluations to identify client needs and develop effective care strategies
  • Communicate effectively with clients, families, and healthcare team members to ensure seamless care coordination
  • Document all client interactions and care provided in a timely and accurate manner
Requirements
  • Current RN licensure in Vermont
  • Minimum one year of recent, verifiable clinical experience
  • Prior home care and/or hospice experience preferred, but not required
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
