Job Summary:
- Provide skilled nursing care to clients in their homes, focusing on comfort, dignity, and quality of life.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Conduct thorough assessments, identify needs, and provide education to clients and their families.
- Administer medications, perform wound care, and provide other treatments as needed.
- Document all interactions, assessments, and care provided in the client's medical record.
Requirements:
- Current license as a Registered Nurse (RN) in Vermont.
- Minimum of one year of recent, verifiable clinical experience.
- Prior home care and/or hospice experience preferred, but not required.
