Medical Director

Family HealthCare Network is seeking a highly skilled and dedicated Internist to join our team of healthcare professionals. As an Internist, you will be responsible for providing primary care medical services in a Patient Centered Health Home context, with a focus on access to care.
Key Responsibilities- Direct Provision of Primary Care Services: Provide examination and review of laboratory, referral, and imaging results to determine a diagnosis. Document visits accurately using appropriate templates and record relevant data to attain a diagnosis.
- Internal Medicine Focus: Provide comprehensive, continuous, and coordinated care managing both common and complex illnesses in adults and the elderly. Utilize chronic disease management skills to provide a team-based, patient-centered approach to the planned care of chronic conditions.
- Excellent Customer Service: Consistently behave with courtesy when interacting with patients, family members, support staff, and Network staff. Provide patients with educational materials and resources appropriate to their health literacy needs.
- Patient Status and Quality Measures: Promote and participate in daily huddles. Incorporate the participation of ancillary health team members in managing the care of patients and families. Set and monitor self-management plans.
- Performance Improvement and Staff Education: Assist the patient care team with performance evaluations and quality improvement. Participate in rotation of medical, physician assistant students, and dental residents.
- Professional and Technical Knowledge: Equivalent of extensive formal training in Internal Medicine, including an understanding of the application of the theory and practices of the profession to the operations of the organization.
- Technical Skills: Ability to create complex documents in Microsoft Word, use advanced functions of Microsoft Excel, and develop sophisticated presentations in Microsoft PowerPoint.
- Licenses and Certifications: Licensed in the state of California as an Internist. Board-eligibility or certification in Internal Medicine may be required or desired in certain positions.
