About the Role:

We are looking for a Physical Therapist to provide high-quality care to adult and geriatric clients in their homes. As a Physical Therapist, you will be responsible for assessing clients' physical abilities, developing and implementing individualized care plans, and educating clients and their families on rehabilitative care and activities.

Responsibilities:
  • Perform diagnostic tests and measurements to assess clients' physical abilities
  • Develop and implement individualized care plans to improve clients' physical function and mobility
  • Provide education and training to clients and their families on rehabilitative care and activities
  • Collaborate with other healthcare professionals to ensure comprehensive care
Requirements:
  • Current Pennsylvania Physical Therapist license
  • Minimum of one year of recent work experience as a Physical Therapist
  • Graduation from a program approved by the American Physical Therapy Association
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred
