Position Overview

The Vistas at Bettendorf is in search of a dedicated Full-Time MDS Coordinator, seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) with strong organizational, leadership, assessment, and documentation capabilities. A background in MDS within a long-term care environment is essential, along with a valid Nursing license in Iowa.

Key Responsibilities
  • Ensure the precise and timely execution of Minimum Data Set (MDS) assessments and care plans for patients.
  • Conduct thorough evaluations of patients' medical, functional, and psychosocial requirements.
  • Collaborate with healthcare professionals to collect necessary medical documentation.
  • Implement clinical documentation improvement strategies to guarantee accurate coding and billing.
  • Work alongside the interdisciplinary team to formulate personalized care plans.
  • Oversee and update patient records in accordance with regulatory standards.
  • Provide training and guidance to staff on MDS completion and documentation best practices.
  • Prior experience in HEDIS reporting and medical documentation is advantageous.
  • Familiarity with hospice care, case management, and utilization management practices is preferred.
  • Comprehension of Diagnosis Related Groups (DRG) and reimbursement methodologies is beneficial.
Qualifications
  • Must hold a current, unencumbered, active state license to practice as an RN or LPN.
  • A minimum of 1 year of MDS experience is required.
  • Experience in Skilled Nursing or Rehabilitation facilities is preferred.
  • Qualified candidates should demonstrate compassion and a commitment to excellence.
  • Education: Associate's Degree is required.
