Key Responsibilities
  • Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being, including educating staff on the role of social services.

  • Hold meetings with patients/residents, families, and significant others as requested to provide support and information to improve quality of life.
  • Perform patient/resident discharge planning activities, including family/patient education, nursing center documentation, and referrals to appropriate agencies and suppliers.
  • Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
