Home Health Nursing Opportunity:

A Path of Care Hospice seeks a skilled Licensed Practical Nurse (LPN) to join our team in providing home health nursing care to patients in their own homes.

Salary and Benefits:

The salary range for this position is between $45,000 and $65,000 per year, depending on skills and experience, plus a generous loyalty bonus of $3,000 and an excellent benefits package that includes medical, dental, vision, life insurance, and a 401(k) plan.

Duties and Responsibilities:

  • Provide personalized care to patients in their homes.
  • Collaborate with RN case managers to develop and implement care plans.
  • Counsel patients and families on advance directive issues.
  • Document patient progress and maintain accurate records.

Requirements:

  • Oklahoma LPN license.
  • 1 year of clinical experience.
  • Ability to care for patients with terminal illnesses.
  • Valid Oklahoma driver's license, reliable transportation, and vehicle insurance.
