Job Summary:

The successful candidate will be responsible for facilitating care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources, balanced with the patient's resources and right to self-determination. This involves assessing patients at risk for readmission, conducting complex psycho-social assessments, and promoting timely throughput, safe discharge, and prevent avoidable readmissions.

Responsibilities:
  • Develop and implement transition plans to promote efficient care sequencing and ensure patients receive the right level of care
  • Assist patients and families with adoption, abuse, and neglect cases, including assessment, intervention, and referral to local, state, and federal agencies
  • Care coordination, including implementation or oversight of transition plan execution
  • Facilitate multi-disciplinary patient care conferences, including Complex Case Review
  • Communicate effectively with patients, families, physicians, office staff, and ancillary departments

Requirements:
  • Master's of Social Work degree required
  • Preferred: 2 years of acute hospital experience
  • Required certifications include MSW and active license in the state(s) covered
  • Accredited Case Manager (ACM) certification preferred
