About the Job

The Patient Advocate Specialist will play a critical role in ensuring that women living with cancer in San Francisco receive the comprehensive care and support they need to thrive.

Job Summary:

  • Provide case management and individualized care planning services to clients with cancer.
  • Advocate for clients' needs within the healthcare system and connect them with community resources.
  • Conduct home visits and provide emotional support to clients in their homes and other community settings.
  • Maintain accurate client records and provide regular progress updates to healthcare providers and community agencies.
  • Collaborate with team members to develop and implement effective care plans that address the unique needs of each client.

Qualifications:

  • Master's degree in social work or a related field.
  • At least 3 years of experience in patient advocacy or a related field.
  • Bilingual Cantonese/English proficiency required; Mandarin proficiency preferred.
  • Excellent communication, organizational, and interpersonal skills.
  • Detailed documentation practices and understanding of privacy compliance.
