Key Responsibilities:
  • Assess and evaluate patients' physical therapy needs and develop individualized plans of care.
  • Provide physical therapy services, including exercises, stretches, and other interventions, to help patients achieve their goals.
  • Collaborate with other healthcare professionals, such as nurses and physicians, to ensure comprehensive care.
  • Conduct home assessments to identify potential safety hazards and provide recommendations for modifications.

Requirements:
  • Current licensure as a Physical Therapist in the state of practice.
