Job Description

Master Social Worker

Innovative Renal Care seeks a skilled Master Social Worker to join our team. As a key member of our dialysis care team, you will play a vital role in helping patients understand their rights and responsibilities, and guiding them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.

Responsibilities:

  • Assess patient needs and develop personalized care plans.
  • Collaborate with patients, families, and healthcare teams to ensure comprehensive care.
  • Provide education and support to patients and their families on disease management and treatment options.
  • Facilitate communication between patients, families, and healthcare providers.
  • Document patient progress and outcomes.

Requirements:

  • Master's degree from an accredited school of social work (MSW).
  • Current state licensure (e.g. LCSW, LMSW), requirements for the state of practice.
  • Two years of clinical work experience, preferably in an ESRD setting.
  • Current CPR certification preferred.
  • Excellent communication and interpersonal skills.
