Responsibilities:
  • Assess patients' needs and develop personalized care plans
  • Collaborate with patients, families, and healthcare teams to ensure comprehensive care
  • Provide education and support to patients and their loved ones
  • Advocate for patients' rights and interests
Requirements:
  • Master's degree in Social Work (MSW) from an accredited institution
  • Current state licensure (e.g. LCSW, LMSW) and meet all state required regulations to practice in a dialysis setting
  • Two years of clinical work experience, preferably in an ESRD setting
  • Excellent communication and interpersonal skills
