We are seeking a highly motivated and compassionate individual to join our team as a Community Care Worker I. In this role, you will work with vulnerable citizens of Texas to determine their eligibility for in-home care, home delivered meals, emergency response services, and other social services.

Key Responsibilities:
  • Conduct home visits and interviews with individuals who are older or have a disability to assess their needs and provide information and referral to other resources.
  • Determine eligibility and functional needs through reviews and assessments.
  • Obtain, verify, and calculate income and resources to determine financial eligibility for Title XX programs.
  • Maintain accurate and up-to-date case records and enter information into an automated system.
  • Monitor services to ensure they are meeting the individual's needs and develop service plans accordingly.
Requirements:
  • Ability to effectively communicate orally and in writing.
  • Ability to set priorities, establish timeframes, and meet deadlines.
  • Ability to establish and maintain effective relationships with individuals receiving services, co-workers, and other stakeholders.
  • Knowledge of interviewing techniques and resources that serve individuals who are older or have a disability.
