Hospice Nurse

Mercy Hospice offers a unique opportunity for skilled nurses to provide exceptional care to individuals at the end of their lives in the comfort of their own homes.
As a Hospice Nurse - Community Care Specialist, you will have the autonomy to provide high-quality, patient-centered care to patients and their families, while also having the opportunity to connect and engage with your community in a meaningful way.
Key Responsibilities:
- Provide skilled professional nursing care to assigned terminally ill patients in their place of residence
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with the interdisciplinary team to ensure comprehensive care and support
- Provide education and support to patients and their families on end-of-life care and bereavement
Requirements:
- Current RN license in the state of Missouri
- BLS CPR Certification through American Heart Association
- Recent hospice or long-term care experience preferred
