We are seeking a highly skilled and compassionate Lung Transplant Nurse Coordinator to join our team at Mass General Brigham. As a key member of our transplant program, you will play a vital role in ensuring the seamless care of our patients throughout their post-transplant journey.
Key Responsibilities- Coordinate and manage the care of patients undergoing lung transplantation, including pre-transplant evaluation, post-transplant follow-up, and ongoing care.
- Collaborate with the multidisciplinary transplant team to ensure comprehensive and coordinated care.
- Develop and implement individualized care plans, taking into account the unique needs and circumstances of each patient.
- Provide education and support to patients and their families, addressing their concerns and promoting optimal health outcomes.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care.
- Graduate of an approved school of nursing with current registration in Massachusetts.
- Minimum 2 years of experience in pulmonary, thoracic surgery, or transplant nursing.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
- Knowledge of community resources and post-acute care providers, with a strong understanding of the complexities of transplant care.
