Role Summary:

  • The Rehabilitation Specialists are tasked with aiding individuals with medical and functional impairments to achieve and regain optimal functionality through a patient-centered care methodology. The therapist assists individuals in fulfilling their treatment objectives and adapting to therapeutic aids by administering manual exercises, guiding, motivating, and supporting patients in engaging in physical activities, including non-manual exercises, mobility tasks, and daily living activities, as well as utilizing assistive devices like crutches, canes, and prosthetics. Collaboration with a multidisciplinary team is essential to realize the targeted outcomes outlined in each individual's care plan, crafted by the rehabilitation physician, with adjustments made based on the patient's response to treatment.

Key Responsibilities:

  • Deliver professional rehabilitation services.
  • Uphold confidentiality standards.
  • Integrate ethical principles into practice, advocating for patient rights and reporting any unethical practices.
  • Evaluate personal practice against professional standards and guidelines.
  • Provide health education tailored to the patient's developmental stage, learning needs, and cultural background, covering topics such as healthy living and preventative self-care.
  • Ensure continuity of care post-discharge by designing home exercise programs and instructing patients and caregivers accordingly.
  • Enhance the quality of rehabilitation practice through participation in performance improvement initiatives.
  • Commit to lifelong learning and professional development.
  • Possess the necessary knowledge and skills to meet the physical, psychological, and social needs of patients.
  • Participate in compliance with established standards and accept additional duties as assigned.

Care Delivery:

  • Conduct comprehensive assessments and document relevant health data systematically.
  • Utilize evidence-based assessment techniques to ensure culturally responsive and holistic patient care.
  • Analyze assessment data to determine diagnoses, validating findings with patients and the healthcare team.
  • Develop individualized care plans that reflect patient characteristics and preferences.
  • Identify expected outcomes for care plans, documenting measurable goals.
  • Coordinate the implementation of care plans, documenting interventions and modifications as necessary.
  • Evaluate progress towards outcomes and revise care plans based on ongoing assessments.
  • Ensure adherence to patient safety and quality standards.
  • Engage in multidisciplinary collaboration for discharge planning.

Leadership and Research:

  • Demonstrate leadership by actively participating in committees and community outreach.
  • Incorporate research findings into practice to inform decision-making.
  • Utilize resources effectively, considering safety, cost, and patient needs.

Team Collaboration:

  • Contribute to the professional growth of colleagues through knowledge sharing and constructive feedback.
  • Delegate appropriate tasks to Physical Therapy Assistants and Aides.
  • Collaborate with the rehabilitation team to establish wellness-oriented goals.
  • Participate in interdisciplinary team meetings to influence care decisions.

Qualifications:

  • Licensed Physical Therapist (Required)
  • Basic Cardiac Life Support certification (Required)
  • Master's Degree in Physical Therapy (Required)
  • Preferred: 2-4 years of experience as a Physical Therapist
