Role Summary:

  • The Rehabilitation Specialists are tasked with aiding individuals with medical and functional challenges to achieve and regain optimal functionality through a patient-centered care model. The therapist assists clients in fulfilling their treatment objectives and adapting to therapeutic devices by implementing manual exercises, guiding, motivating, and supporting patients in engaging in physical activities, including non-manual exercises, mobility tasks, and daily living activities, as well as utilizing assistive devices like crutches, canes, and prosthetics. Collaboration with a multidisciplinary team is essential to meet the desired outcomes outlined in each individual’s care plan, which is developed by the rehabilitation physician, with adjustments made based on the patient's response to treatment.

Key Responsibilities:

  • Delivering Professional Rehabilitation Services
  • Upholding Confidentiality: The Rehabilitation Specialist adheres to confidentiality standards.
  • Ethical Practice: The Specialist incorporates ethical principles into practice, advocating for patient autonomy, dignity, and rights, while reporting any illegal or incompetent practices.
  • Self-Evaluation: The Specialist assesses their own practice against professional standards and regulations.
  • Health Education and Promotion: The Specialist provides health education tailored to the patient's developmental level, learning needs, and cultural background, covering topics such as healthy living and preventative self-care.
  • Post-Discharge Care: The Specialist ensures continuity of care by designing home exercise regimens and instructing patients and caregivers on these programs.
  • Quality Improvement: The Specialist actively participates in initiatives aimed at enhancing the quality and effectiveness of rehabilitation services.
  • Commitment to Education: The Specialist engages in lifelong learning to maintain and enhance their skills and competencies.
  • Comprehensive Patient Care: The Specialist possesses the necessary knowledge and skills to address the physical, psychological, and social needs of patients.
  • Compliance and Additional Duties: The Specialist participates in compliance activities and accepts other responsibilities as assigned.

Care Delivery:

  • Assessment: The Specialist systematically collects and documents comprehensive data relevant to the patient's health.
  • Diagnosis: The Specialist analyzes assessment data to identify diagnoses or issues, validating these with the patient and healthcare team.
  • Planning: The Specialist formulates and documents a personalized care plan that reflects the patient's characteristics.
  • Implementation: The Specialist coordinates the execution of the care plan, documenting interventions and modifications as necessary.
  • Evaluation: The Specialist assesses and documents progress towards achieving care goals, making adjustments based on ongoing evaluations.
  • Accountability: The Specialist is responsible for enhancing patient care through adherence to safety and quality standards.
  • Leadership and Research: The Specialist demonstrates leadership by participating in committees and community outreach, integrating research findings into practice.
  • Resource Management: The Specialist effectively utilizes resources to ensure optimal patient care.

Team Collaboration:

  • Professional Development: The Specialist contributes to the growth of colleagues by sharing knowledge and providing constructive feedback.
  • Delegation: The Specialist appropriately delegates tasks to support staff based on their competencies.
  • Collaboration: The Specialist works with the rehabilitation team to set realistic wellness goals for patients.
  • Interdisciplinary Participation: The Specialist engages in team meetings, contributing to collaborative decision-making.

Qualifications:

  • Licensed Physical Therapist (Required)
  • Basic Cardiac Life Support Certification (Required)
  • Master's Degree in Physical Therapy (Required)
  • Two to four years of experience as a Physical Therapist (Preferred)
