ResponsibilitiesPurpose: Engage in all aspects of pharmaceutical services, encompassing medication distribution and clinical interventions, while honoring the dignity of all individuals and ensuring cost efficiency.
Key Duties:
- Conduct comprehensive medication reviews and provide recommendations to healthcare professionals.
- Collect and analyze PharmDoc intervention data regularly.
- Participate actively in renal dosing protocols.
- Evaluate medication orders for accuracy regarding dosage, frequency, indications, and potential drug interactions.
- Guide nursing staff in resolving medication-related issues.
- Address and manage tasks in the Pharmacy Order Entry System promptly.
- Monitor and register Clozapine patients in IVAX.
- Be flexible to cover various pharmacy areas as needed.
- Engage in pharmacokinetic monitoring and TPN calculations for adults and pediatrics.
- Contribute to anticoagulant management and pain management services.
- Participate in microbiology culture monitoring and IV to PO conversion programs.
- Prepare and verify intravenous admixtures.
- Provide drug information and counsel patients on their medication regimens.
- Document inquiries in the drug information database as per policy.
- Participate in adverse drug reaction (ADR) and medication error reporting processes.
- Propose enhancements for safety in medication use.
- Manage formulary processes and conduct medication reviews.
- Complete a Drug Utilization Evaluation (DUE) project annually.
- Present findings to the Pharmacy & Therapeutics Committee yearly.
- Support formulary management initiatives.
- Contribute to policy development and revisions within the department.
- Educate staff on new or updated policies and procedures.
- Maintain professionalism and leadership standards through ongoing education and training.
- Act as a preceptor for students or residents.
- Engage in clinical research and departmental education initiatives.
- Complete all job-specific competencies, including age-specific training.
Education: Must be a graduate of an accredited College of Pharmacy with at least a Bachelor’s Degree in Pharmacy.
Licensure: Must hold a valid pharmacist license in good standing in the State of North Dakota.
Experience: No prior experience required.
Skills: Proficient in computer applications and capable of providing pharmaceutical care across all age groups.
Interpersonal Skills: Ability to communicate effectively with staff, patients, and visitors while maintaining confidentiality.
Other: Exposure to environmental hazards typical of a medical facility. Participation in a Hepatitis B vaccination program is available. Occasional exposure to hazardous materials and mental/visual strain may occur. Commitment to continuing education as mandated by the North Dakota State Board of Pharmacy is essential.
