About the Position

Key Accountabilities
  • Develop and maintain relationships with patients, families, and healthcare providers to ensure seamless communication and coordination of care.
  • Identify and address the unique needs and concerns of patients and their families, providing personalized support and guidance.
Requirements

To succeed in this role, you will need:

  1. A Bachelor's Degree.
  2. Two years of experience in a health or mental health setting (desired).
  3. The ability to empathize with patients and families, and provide compassionate support (essential).
