The Klein Family Harford Crisis Center is a comprehensive, trauma-informed behavioral health hub that utilizes innovative evidence-based modalities to serve our community. Our center was created with Centers for Disease Control (CDC) recommendations in mind to address the behavioral health crisis in Harford County, Maryland. We offer a range of services, including an Urgent Care Walk-In Clinic, an Outpatient Therapy/Psychiatry Clinic, and short-term Residential Crisis Beds, to provide comprehensive care to those in need.
Job SummaryThe Behavioral Health Clinician II acts as an extension of the psychiatric provider, working collaboratively with the behavioral health team, medical team, and community stakeholders to provide comprehensive care. Key responsibilities include completing psychosocial assessments, developing treatment plans, monitoring patient progress, and crisis de-escalation. The ideal candidate will have a Master's Degree in Human Services and be licensed as an LCSW-C or LCPC in the state of Maryland.
Responsibilities- Complete psychosocial assessments to evaluate patient needs and develop treatment plans.
- Work collaboratively with the behavioral health team and medical team to provide comprehensive care.
- Develop and implement discharge and aftercare plans to ensure continuity of care.
- Coordinate services with community providers, including knowledge of community resources, Medicare/Medicaid, and commercial insurances.
- Provide trauma-informed individual, group, and family therapy services to patients.
- Initiate coordination of discharge and aftercare services as needed.
The ideal candidate will have a Master's Degree in Human Services and be licensed as an LCSW-C or LCPC in the state of Maryland. Additionally, the candidate should have knowledge of behavioral health clinical practice, including psychosocial assessments, trauma-informed therapy, and discharge planning.
