Job Summary

We are seeking a highly skilled RN Care Manager to join our team. The successful candidate will be responsible for planning and prioritizing care for individuals and populations of patients, focusing on strategies that promote optimal health within populations.

Key Responsibilities

  • Assess internal and external referrals to identify patient/significant others' needs, level of intensity, insurance benefits, and other patient resources.
  • Develop and implement care plans, making recommendations to PCPs, specialists, and other members of the healthcare team regarding care management strategies.
  • Assess the educational needs of patients, families, and healthcare team members, developing and implementing teaching strategies or making appropriate referrals.
  • Communicate and collaborate with patients, significant others, providers, and payers to coordinate services that improve access to appropriate services across the continuum of care.
  • Measure patient care outcomes, interpret reports, and analyze data trends for groups of patients.
  • Document patient data, plans, interventions, and outcomes according to department guidelines.

Qualifications

  • Required: Bachelor's Degree or equivalent in nursing or a healthcare-related field.
  • Required: 2 years of relevant experience in clinical nursing and/or case management/managed care or a related field.
  • Preferred: 5 years of relevant experience in clinical nursing and/or case management/managed care or a related field.
  • Required: Registered Nurse (RN) - State of Michigan.
  • Required: Basic Life Support (BLS) - AHA American Heart Association or ARC American Red Cross.
