About the Role

Responsibilities
  • Implementing Care Plans: Develop and implement individualized care plans in collaboration with our patients' physicians and healthcare teams.
  • Patient Assessments: Conduct thorough assessments of our patients' physical and emotional needs, identifying areas for improvement and developing strategies for optimal care.
  • Medication Administration: Administer prescribed medications, treatments, and therapies as ordered by our patients' physicians.
  • Documentation: Maintain accurate and timely documentation of patient care, including progress notes, medication administration records, and other relevant information.
  • Health Education: Educate patients and their families on healthy habits, disease management, and other relevant topics to promote optimal health outcomes.
  • Communication: Communicate effectively with our patients, their families, and our healthcare teams to ensure seamless care coordination and address any concerns or issues that may arise.
Requirements
  • Licensure: Must have and maintain an active, unencumbered LPN license in the state where you will be practicing.
  • CPR Certification: Current CPR certification from the American Red Cross or American Heart Association.
  • TB Skin Test: Must have a current TB skin test within the last 12 months.
  • Experience: Six months of prior hands-on nursing experience preferred, but not required.
  • Transportation: Must have reliable transportation to and from patient visits.
