Transformative Healthcare Provider Consultant

We are seeking a visionary Healthcare Provider Consultant to lead transformative initiatives in care coordination and consulting. Key Responsibilities:
  • Strategic Care Planning and Optimization: Develop and implement strategic care plans tailored to meet the diverse needs of our clients, leveraging data analytics and best practices to optimize outcomes and resource utilization.
  • Innovative Care Coordination Solutions: Explore and implement innovative care coordination solutions, such as telehealth initiatives, predictive analytics, and community partnerships, to enhance the accessibility and effectiveness of care delivery.
  • Leadership in Provider Engagement: Provide visionary leadership in building and nurturing relationships with healthcare providers and community stakeholders, driving collaboration and alignment towards shared goals of improving patient care and outcomes.
  • Continuous Quality and Performance Improvements: Establish robust quality improvement processes and leverage performance metrics to monitor and evaluate the effectiveness of care delivery, driving continuous improvement and ensuring accountability for outcomes.
  • Thought Leadership and Knowledge Sharing: Serve as a subject matter expert in value-based care and care coordination, contributing to thought leadership initiatives, publications, and industry conferences to advance the field and promote organizational expertise.
  • Mentorship and Team Development: Provide mentorship and guidance to peers and junior staff members, fostering a culture of learning and professional growth within the organization.
  • Strategic Planning and Market Growth: Network with providers and provider groups in assigned geographic markets to promote and foster growth of the Accountable Care Organization (ACO).
  • Population Health Initiatives: Assist in directing multiple population health initiatives/projects simultaneously within a healthcare network and educate providers and support staff in these initiatives.
  • Data-Driven Decision Making: Possess general knowledge of data analytics and leverage data to make recommendations based on best practices to improve clinical and administrative workflows.
  • Operational Excellence: Conducts routine bimonthly meetings with the staff (including PCP's) and documents topics discussed, action plans, and staff feedback.
  • Compliance and Governance: Support company philosophies, objectives, decisions, and policies, and ensure adherence to all HIPAA, Confidentiality, and Privacy laws.
  • Performance Metrics Management: Manage performance metrics for the department.
Requirements:
  • Bachelor's degree in Healthcare Administration, Nursing, Public Health, or a related field required. (Alternatively, 4-6 years of significant and relevant work experience in medical practice management may be accepted in lieu of educational qualifications, especially with extensive administrative experience in a clinic setting.)
  • Residency in San Diego required, with the ability to travel up to 25% within the state.
  • Preferred experience with large multi-site practices.
  • Strong oral and written communication skills, including public presentation, organizational, project management, and team-building abilities.
  • Proficiency in process mapping and workflow analysis tools.
  • Experienced in using Electronic Health Records (EHR) for clinical processes.
  • Advanced computer skills and a willingness to learn new software applications.
  • A holistic thinker who contributes to the overall improvement of Advanced Management, through active participation in diversity and inclusion efforts, culture initiatives, facilitating training, and leading roundtables.
  • Demonstrated leadership experience with the ability to inspire and motivate cross-functional teams towards a shared vision of excellence in care delivery.
  • Entrepreneurial mindset with a passion for identifying and seizing opportunities for growth and improvement.
  • Exceptional communication and interpersonal skills, with the ability to effectively engage and influence stakeholders at all levels.
  • Deep understanding of value-based care principles, healthcare policy, and industry trends.
  • Strong analytical skills and proficiency in data-driven decision-making.
  • Must be a proven self-starter, possessing proactive initiative, and resourcefulness.
  • Proven interpersonal skills with the ability to interface effectively both internally and externally with a wide range of people including physicians, office staff, hospital executives, medical groups, IPA's, community organizations, and other health plan staff.
  • Must demonstrate proficiency in Microsoft Excel, Word, and PowerPoint Presentations.
  • A high level of engagement and emotional intelligence.
