We are seeking a highly skilled Care Coordinator to join our team at Meridian Illinois. As a Care Coordinator, you will play a critical role in developing and implementing personalized care plans for our elderly and physical disability waiver members.
Key Responsibilities- Evaluate the needs of our members and recommend the best course of action to ensure quality, cost-effective healthcare outcomes.
- Develop and implement ongoing long-term care plans and work with healthcare providers and community resources to ensure identified services are accessible to members.
- Provide education and support to members and their families/caregivers on procedures, healthcare provider instructions, service options, referrals, and healthcare benefits.
- Monitor care plans and member status, providing recommendations for improvement and ensuring compliance with state, federal, and third-party payer regulators.
- Perform on-site visits to assess member needs and collaborate with providers or resources as needed.
- Bachelor's degree in a human services-related field or equivalent experience.
- 1 year of related experience in conducting comprehensive assessments and provision of formal services for the elderly.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work independently and as part of a team, with a focus on delivering high-quality care and services.
