Key Responsibilities- Provide comprehensive nursing care to patients in their homes, including assessments, medication management, and wound care.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Conduct thorough patient assessments, identifying needs and developing strategies to address them.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and coordination.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal care outcomes.
- Current RN licensure in the Commonwealth of Virginia.
- One year of recent RN experience, with preference given to those with Home Health experience.
- Ability to independently drive to and from patient homes.
- Positive attitude, enthusiasm for caring for our community, and strong verbal and written communication skills.
