Position Title: Case Manager I

Department: All Sites

Reports To: Community Supports Program Operations Supervisor

POSITION SUMMARY

The Case Manager I plays a crucial role in delivering comprehensive case management services to individuals within their assigned caseload. This position focuses on assisting clients in their journey from homelessness to stable housing, or supporting those who have previously experienced homelessness in maintaining their housing stability. The ideal candidate will embody a client-centered philosophy, demonstrating a commitment to providing exceptional service that acknowledges the diverse challenges faced by individuals with various medical and behavioral health conditions.

Key Responsibilities:

  1. Conduct outreach and engagement activities.
  2. Perform housing assessments and develop Individualized Housing Support Plans.
  3. Facilitate goal setting and address barriers to housing.
  4. Provide housing and rental assistance.
  5. Link clients to health, mental health, and substance use disorder services.
  6. Monitor ongoing client progress and provide follow-up support.
  7. Assist clients in establishing benefits and provide transportation support.
  8. Manage crisis situations and prevent eviction.
  9. Educate clients and landlords about housing processes.
  10. Assist in locating suitable housing options.

The successful candidate will have prior experience in housing case management and a strong understanding of local resources and community organizations. Excellent communication and problem-solving skills are essential for addressing client needs effectively. Bilingual candidates are encouraged to apply.

SALARY RANGE:

$29.50 per hour, dependent on experience.

WORK SCHEDULE:

8 hours per day, 5 days per week.

GENERAL DUTIES:

  1. Maintain professionalism in all interactions.
  2. Support the mission and objectives of the Agency.
  3. Adhere to professional ethics and policies.
  4. Inform clients about additional services available through the organization.
  5. Participate in training and educational workshops.
  6. Attend all required staff meetings.
  7. Collaborate with team members to develop comprehensive treatment plans.
  8. Assist with vocational referrals and job readiness support.
  9. Provide outreach and follow-up services to clients.
  10. Facilitate discharge planning for clients, addressing all relevant areas.
  11. Engage with internal and external entities to resolve client issues.
  12. Document client progress and case management activities in medical records.
  13. Perform additional duties as assigned by management.

EXPERIENCE REQUIREMENTS:

  1. For AOD Programs: Enrollment in an academic Addiction Studies program and registration with relevant certification bodies.
  2. For all other Programs: Minimum of 1 year experience in a Behavioral Healthcare setting.
  3. Bilingual English/Spanish preferred.
  4. Valid California driver's license and insurance required.

LICENSES/CERTIFICATES:

For AOD programs, registration or certification with a DHCS approved state-level counselor certification agency is required.
