Family Nurse Practitioner

CVS Health is seeking a highly skilled and compassionate Family Nurse Practitioner to join our team. As a Family Nurse Practitioner, you will provide high-quality, patient-centered care to our patients, working collaboratively with our healthcare team to deliver exceptional outcomes.
Responsibilities:- Provide primary and specialty care to patients of all ages, including routine check-ups, health screenings, and treatment of acute and chronic conditions.
- Conduct thorough health assessments, develop and implement individualized care plans, and provide education to patients and their families.
- Order and interpret diagnostic tests, prescribe medications, and provide referrals to specialists as needed.
- Collaborate with other healthcare professionals to coordinate patient care and ensure seamless transitions between settings.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and safety.
- Master's degree in Nursing with a focus on Family Nurse Practitioner or related field.
- Current licensure as a Registered Nurse and certification as a Family Nurse Practitioner.
- Minimum of 2 years of experience as a Family Nurse Practitioner, preferably in a primary care setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
