Job Summary

The Home Care Coordinator is a vital member of our team, responsible for organizing the plan of care for participants while providing education and eliminating any potential barriers to treatment. This role requires a skilled occupational therapist to work under the general supervision of the Clinical Director.

Key Responsibilities
  • Provide skilled occupational therapy in accordance with the plan of care
  • Assist the IDT in the evaluation and assessment of the patient's functional level by applying appropriate diagnostic and prognostic procedures and tests
  • Recommend orthotics as indicated by condition and needs
  • Treat the participant through the use of therapeutic activities designed to restore function and self-care activities for the purpose of improving function
  • Observe, record, and report the participant's reaction to treatment and any changes in the participant's condition to the physician, PACE Clinical Director or designated, or IDT as appropriate
Requirements
  • Registered by the American Occupational Therapy Association or successfully completed the educational requirements of occupational therapy
  • State of Washington Occupational Therapist license
  • Health Care Provider level CPR certification
  • Minimum one (1) year of occupational therapy experience
  • Minimum one (1) year of experience working with frail or elderly population preferred
  • Experience in Medicare Certified Home Health preferred
